We offer new and returning patients the choice to download our required forms in advance of appointments so you may fill them out at home at your convenience.

Click on the appropriate section below and you can download and print the forms. Don’t forget to bring them to your next appointment.

New Patients

Please complete all of the required forms. If you are suffering from both back and neck pain, please complete both disability index forms.

• New Patient Paperwork (required for all new patients)

• Back Pain Disability Index (Oswestry) (required for back pain patients)

• Neck Pain Disability Index (Oswestry) (required for neck pain patients)

Returning Patients

Please complete all of the required forms, including the forms for your specific physician. These forms are required for each appointment in our office, and they help our doctors track your recovery and progress.

If you are suffering from both back and neck pain, please complete both disability index forms.

• Patient Information Intake Update form (required for all returning patients at each visit)

• Follow up Intake Questionnaire Dr. Hayes (required for Dr. Hayes’ patients at each visit)

• Follow up Intake Questionnaire Dr. Lee (required for Dr. Lee’s patients at each visit)

• Follow up Intake Questionnaire Dr. Saldin (required for Dr. Saldin’s patients at each visit)

• Back Pain Disability Index (Oswestry) (required for back pain patients at each visit)

• Neck Pain Disability Index (Oswestry) (required for neck pain patients at each visit)

Thank you for your cooperation. These forms are also available in our office if you prefer to complete them in person. Please arrive at least 30 minutes prior to your appointment time for registration and check-in.
